EZ-FMLA lets patients tee up complicated FMLA and other medical forms for their physicians. You do not need Hero EMR to use EZ-FMLA. It is free to use and runs on Hero EMR's HIPAA-compliant systems.

Patient chooses form

Start with the FMLA packet or other supported medical form that needs to be completed.

Patient logs into MyChart

The patient connects their Epic MyChart account so the right chart data can be pulled in.

Form fills automatically

EZ-FMLA uses the chart information to populate the packet and tee it up for review.

Physician reviews and signs

The prepared form is routed to the physician for review, approval, and signature.

Patient gets it back

If approved, the finished form is returned to the patient for printing.
